All patients over the age of 16 have full access to our gym, pool and locker room facilities on the day of scheduled therapy visits. We encourage you to take advantage of this unique opportunity to get healthy.

+ Here’s What to Expect on Your First Visit…

Upon arrival, we will ask you for the following:

    1. Prescription for Physical Therapy

    2. Prescriptions are required and must be dated within 30 days of appointment.

    3. Insurance Referral (if required)

    4. Insurance Card

    5. Co-payments: Due on the date of service.

      • We accept cash, checks, and all major credit cards.

    6. Please plan to arrive 15 minutes prior to your first appointment time to fill out necessary paperwork.

+ INSURANCE GUIDE

The set dollar amount that a patient must pay at the time of service. The amount of a patient's copay may vary depending on the type of visit.

The deductible is the amount you pay out of pocket before your insurance will cover any portion of your physical therapy visits. Deductibles reset at the beginning of your benefit period.

The percentage of costs of a covered health care service you pay after you've paid your deductible.
Here is an example:
If your health insurance plans allowed amount for an office visit is $100 and your coinsurance is 20%.
If you have paid your deductible: You pay 20% of $100, or $20. The insurance company pays the rest.
If you haven't met your deductible: You pay the full allowed amount, $100.

Physical Therapy Prescription:
A written order given to you by your doctor to receive Physical Therapy.
The prescription can come from your PCP, Specialist, Ortho, Physician's Assistant or Nurse Practitioner.
Most doctors will write the prescription for a certain number of weeks/visits. Some will leave it up to the therapist to determine a POC (Plan of Care).
RX = Written by the Doctor -> Patient -> Brought to Appointment for Therapist

Insurance Referral:
An insurance referral is required by some insurance plans. Most often this will be HOM Plans.
Referrals can only come from your Primary Care Physician.
If you're unsure if your insurance requires a referral, contact your insurance company.
Referral = Processed by Your PCP -> Insurance Company -> Therapist/Practice

We will need you to provide us with the insurance company your claim is open with, a date of injury, the claim number and the name of the adjuster assigned to your case.
We do not bill your personal health insurance unless your claim is denied and you choose to go through health insurance unless your claim is denied and you choose to go through health insurance.

We will need you to provide the name of the Motor Vehicle Insurance, claim number, date of accident and a copy of your insurance card. We will need the name and address of your lawyer, if indicated.

+ PATIENT FAQ’s

All patients must check in at the main front desk of the gym (located on the second floor). You may take the stairs or the elevator up to the desk and the staff will direct you to the office.

Please wear comfortable or loose clothing (shorts, sweats, t-shirts, sneakers). Locker rooms are available for changing/showering. We are a fragrance free environment and ask that no perfume or cologne.

Appointments typically last 40-60 minutes.

On the days of your appointments either two hours before or two hours after, you have access to the fitness center and pool areas. (You may not leave and come back)

Yes! All patients must see a doctor prior to coming in for their first appointment. A prescription will include your diagnosis, frequency, and duration of therapy, specific protocols or treatments, and/or safety precautions from the doctor.

You’ll need a written order from your primary care doctor for you to see a specialist or, to get certain medical services. All HMO plans require a referral, please contact you primary care doctor prior to your first appointment to obtain this.

+ CANCELLATION/RESCHEDULING

• Any appointments not canceled within 24 hours are subject to a $35 late cancel/no show fee.

• We reserve the right to reschedule any appointments.

+ POOL THERAPY HOURS:

7:00 am – 5:00 pm

8:00 am – 2:00 pm

+ WEATHER RELATED CLOSINGS

During snow or inclement weather we ask that you please call the office prior to coming in for your appointment. Our website and voice mail will be updated with changes to our hours, during inclement weather.
